Membership Application Form

To become a member, please fill out the form below and we will be in contact within the next 48 hours.

  • there is a $50 joining fee for all new & renewal memberships

  • all memberships require a credit/debit card to be on file

  • monthly EFT charges will be made on the 5th of the month

  • couples memberships consist of you and your spouse/partner living at the same address

  • family memberships consist of you, your spouse/partner and dependents under 19 living at the same address

  • ball machine is not included in the membership. You can choose to add this option to your membership below. Ball Machine is only available for tennis & use is only available to members.

  • cancellations must be made before the 1st of the month to not incur the next month’s charge & will be subject to an additional joining fee at the new membership rate (cancel by filling out the Cancellation Form below).

Membership Cancellation Request Form

  • cancellation request must be submitted before the 1st of the month to not incur the next month’s EFT charge

  • by cancelling your membership, you will be subject to a rejoining fee of $50 and new membership rates will apply

  • a confirmation email will be sent to notify you of completed membership cancellation. If you do not receive a confirmation email, please contact sandra@bushtenniscenter.org
